Bruising after blepharoplasty sits in the most visible place on the face, and it is the part of eyelid surgery recovery that people plan their calendar around. The StatPearls entry on upper blepharoplasty lists cold packs for the first 24 hours, sleeping with the head raised for a week, and no strenuous activity for two weeks as the standard measures for swelling and bruising. This article covers why the eyelids bruise, how the bruising usually runs, and what the Hyperbaric Oxygen Therapy (HBOT) evidence in facial surgery shows. No published study is specific to blepharoplasty. The bruising data come from facelift patients and are described below with their limits.
Eyelid skin is the thinnest on the body and sits over loose tissue with a dense supply of small vessels. Vessels cut or stretched during surgery leak blood into that loose tissue, and because the skin over it is thin, a small amount of blood shows as a large bruise. Gravity pulls the blood downward, which is why lower-lid bruising can track onto the cheek in the first days.
Bruising darkens over the first two or three days, then changes color as the blood breaks down and is cleared, and it typically fades over one to two weeks. Swelling of the lids can outlast the color. Your surgeon's instructions on cold packs, head elevation, activity, and any prescribed medication set the schedule for your case.
One small study has directly measured facial bruising with HBOT, and it was in facelift patients. The rest of the evidence is indirect.
Stong and Jacono (2010) : 13 facelift patients, 6 of whom chose five 60-minute sessions at 2 atmospheres, two before surgery and three on days 3 to 5 after. Cheek bruising was measured by color analysis of standardized photographs. The HBOT group showed 35% less color change on day 7 (P = .005) and 30% less on day 10 (P = .03), with no difference on days 1 and 5. All patients in both groups also took oral steroids, arnica, and bromelain, and the study was not randomized.
Neel et al. (2023) : a case-control study of 20 facelift patients in which wound healing took a mean of 13.3 days with HBOT and 36.9 days without. Bruising and swelling were not measured, so it supports faster wound closure only.
Oyaizu et al. (2018) : a rat study of muscle bruising in which two hours a day at 2.5 ATA lowered limb swelling at 6 hours, 24 hours, and 3 days after injury and reduced leakage from small vessels. This is the mechanism behind the interest, measured in an animal limb, not in eyelid skin.
NCT01605110 : a randomized, sham-controlled trial of HBOT for bruising and swelling after upper blepharoplasty, registered in 2012. It never reported results. It is listed here because it is the only eyelid-specific study designed, and because its absence is the reason no eyelid number appears above.
The mechanism is the same one described in the facelift study: at 2.0 ATA, substantially more oxygen dissolves into blood plasma, and the vasoconstriction that accompanies it is the proposed route to less fluid and faster clearance in bruised tissue. Whether the facelift finding transfers to eyelids has not been tested.
