Shock loss is the shedding of transplanted and surrounding hair in the weeks after a hair transplant. For many patients it is the most feared part of the process, because for a stretch of the recovery you can look thinner than you did before the procedure. It is temporary in most cases, and it may be reducible: a 34-patient clinical trial reported a postoperative shedding rate of 27.6% with Hyperbaric Oxygen Therapy (HBOT) versus 69.1% without it.

Shock loss, or postoperative effluvium, refers to the shedding of hair that can occur in the weeks following a hair transplant. This can affect both the transplanted hair grafts and the natural hairs around the transplant area. Medications to Promote Hair Health: Certain medications may also help prevent shock loss by promoting a favorable environment for the hair follicles. Low-dose oral Minoxidil is commonly recommended. Minoxidil works by promoting blood flow to the scalp and directly stimulating the hair follicles, which can help transplanted and surrounding hair enter the growth phase faster. It is also commonly used to support existing hair. However, topical Minoxidil is usually not recommended until at least a month after the surgery, as applying it too soon can interfere with the healing process of the scalp and grafts. Always consult with your surgeon before starting any topical treatments.
Low-Level Laser Therapy (LLLT): Low-Level Laser Therapy (LLLT) is another option that has been studied for improving hair transplant outcomes. The laser treatment helps stimulate circulation to the scalp and energizes follicles, potentially reducing the risk of shock loss. LLLT works by delivering photons into scalp tissue, which increases cellular activity and improves follicle function, helping to maintain the growth phase. LLLT can be administered through in-office treatments or at-home laser caps, making it accessible to many patients.
Hyperbaric Oxygen Therapy (HBOT) for Optimized Healing: Hyperbaric Oxygen Therapy has been studied for reducing shock loss after hair transplants. HBOT involves breathing concentrated oxygen in a pressurized chamber, typically at 2.0 atmospheres absolute (ATA). The increased pressure dissolves more oxygen into the bloodstream, allowing greater delivery to healing tissue.
The proposed mechanism is angiogenesis: new capillaries forming around the transplanted follicles, improving blood flow, nutrient delivery, and oxygen supply. The added oxygen is thought to support follicles through the period when they would otherwise shed.
A clinical trial and a subsequent evidence review have examined HBOT around hair transplants:
Fan, Zhe-Xiang et al. (2021) : a clinical trial of 34 patients receiving FUE transplants. The HBOT group shed less in the early period (27.6% vs. 69.1%), had less itching and folliculitis (11.8% vs. 35.3%), and reported higher early postoperative satisfaction (88.2% vs. 52.9%). At nine months, graft survival did not differ significantly between the groups (96.9% vs. 93.8%).
Parnis, Fenech Magrin, and Hassan (2024) : an evidence review of HBOT across aesthetic practice covering 17 human studies. On hair transplants it reports the trial above, and the authors recommend larger randomized trials with longer follow-up before drawing firm conclusions.
That early difference is what most patients care about: keeping more hair through the shedding window instead of looking thinner while you wait for regrowth. The trial's nine-month follow-up found similar graft survival in both groups, though a 34-patient trial with a single follow-up point is thin evidence in either direction. Whether follicles that avoid the early shed do better over the long term is an open question.
HBOT is not FDA-cleared for hair transplant recovery; the studies above are the current evidence. Discuss it with your surgeon to see if it fits your recovery plan.
